This article proposes the use of an informed consent template for educating social work students. The elements of informed consent, derived primarily from the National Association of Social Workers' "Code of Ethics", are operationalized in the template. The suggestion is that instructors will use the template for classroom instruction and require students to develop an informed consent specific to the students' area of practice. (Contains 1 figure.)
