In the past ten to fifteen years the problem of providing enough cadres for health care has become substantially worse. According to these authors, the primary reason that this has happened is that the system of mandatory job assignment of graduates of educational institutions has been abolished. The entire system of organizing cadre training, in particular for the budget-funded sphere, was not ready to eliminate the centralized job assignment system. Young specialists are definitely not inclined to go out into the countryside to work, because of the wholesale deterioration of social and economic conditions in rural areas and the lower standard of living of medical workers in the countryside. There is also less opportunity to upgrade their qualifications and to associate professionally with colleagues.
