Due to the major advances in technology and the sciences, advances in the medical treatment options for children with chronic conditions are being made at an astonishing rate. In the health care field, "children with chronic conditions" is a generic phrase that typically refers to children with physical, developmental, behavioral, or emotional conditions that last longer than 3 months. These conditions indicate a need for specialized health care, therapies, or medical equipment/supplies, which often causes chronic disruptions in the child's activities when compared with those of children who are typically developing. Conditions that fit this criterion include: diabetes, asthma, behavioral and cognitive disorders, epilepsy, and conditions that require complex health care procedures. While these children may require specialized technological health care procedures and/or health support during the school day, they do not necessarily require special education services (Sirvis, 1989). In many cases, attending school can be a daunting and frightening experience for children with chronic health care conditions, their parents, and school personnel. Communication between the health care team and school personnel is important in the school entry or reentry of the child with a chronic condition (Ishibashi, 2001). Individual Health Plans (IHPs) are a clear and efficient tool that may be utilized to facilitate communication among the myriad of individuals participating on the child's multidisciplinary team.
