This guide details a framework for supporting the efforts of home, school, and community partners to improve continuity and transition in early childhood. Following an introduction describing continuity in early childhood, the importance of a smooth transition, and the eight elements of early childhood continuity, the guide is presented in eight parts, each part corresponding to an important element of continuity: (1) families as partners; (2) shared leadership; (3) comprehensive and responsive services; (4) culture and home language; (5) communication; (6) knowledge and skill development; (7) appropriate care and education; and (8) evaluation of partnership success. Each element is introduced with a rationale describing key concepts from research supporting the element. Indicators are listed for each element, and accompanying worksheet-type pages allow home, school, and community partnerships to consider the meaning and application of each indicator in their work and identify practices in need of change as well as effective practices. A list of resources is included in each of the eight sections, providing further information about the element. The guide concludes with a glossary and guidelines for using this framework to further community efforts. (KB)
