This final report discusses the outcomes of Project STEPS (Sequenced Transition to Education in the Public Schools), an outreach project designed to provide training and technical assistance to states and territories in the development of a statewide transition system for children with disabilities, birth through 5 years of age. The focus of the project is to provide assistance in the development of state policy, a statewide training and technical assistance network, and model local sites for the purpose of establishing a community-wide interagency transition system using the STEPS model and adopted state policies. Outcomes of Project STEPS included: (1) successful facilitation of the development of a statewide transition system in the states of Kentucky, Tennessee, Georgia, North Carolina, and Delaware; (2) provision of liaison support to transition coordinators in states that implemented the STEPS model and funded positions to institute the efforts initiated through this project; (3) hosting a transition forum on an annual basis that included state transition teams from 13 states and territories; and (4) development and dissemination of an annual report from the transition forum that describes issues and strategies for state system development.
