As part of a high-risk youth grant from the U.S. Department of Health and Human Services, Center for Substance Abuse Prevention, a survey was developed and implemented to assess knowledge, attitudes, and behaviors of seventh-grade students toward alcohol, tobacco, and other drugs (ATODs). In addition to assessing project outcomes, this activity provided project staff and school administrators with previously unknown information that can be useful to other local ATOD abuse prevention projects. To that end, the procedures and instrument are provided in this document to assist others in assessing the levels of ATOD use among seventh-grade students. Three appendices provide English and Spanish versions of the survey and of a sample initial consent letter to parents. (Author/SR)
