This Wisconsin study responded to a state law (1989 Wisconsin Act 82) requiring evaluation of the fiscal and programmatic impact of limiting the involvement of school district employees, other than health care professionals, in providing specialized physical health care services to pupils. Responses of 495 (out of 1,000) schools surveyed found the following types of health services being rendered: administration of medications; feeding; assistance with elimination; respiratory care; monitoring of health status; and assistance with mobility. Non-health care professionals providing these services included regular and special education teachers, teacher aides, health aides, special education program aides, school secretaries, and volunteers. Schools reported that factors affecting selection of school personnel to provide specialized health care included the physical health of the pupil, content of the medical orders, and parent requests. Such decisions were most often made by school administrators, school or public health nurses, parents, the child's physician, and teachers. Stressed is the legal requirement that non-health care professionals provide such services only under the direction of a physician or registered nurse. Liability aspects for teachers and registered nurses are considered. Options for school districts considering limiting teacher involvement are presented.
