A Flexible System for Processing Clinical Performance Ratings: Illustrative Applications in a Residency and Four Clerkships.

The Performance Rating portion of the University of Arkansas for Medical Sciences (UAMS) Objective Test Scoring and Performance Rating (OTS-PR) system is designed to help the clinical teacher evaluate students' clinical performance. The system collects, processes, and reports ratings of learners' performances in clinical settings. The system's 160 modular FORTRAN programs are presently run on a mainframe computer, but since the user interacts with an Assessment Scoring Coordinator not the computer, the user (e.g., course director) need have no technical knowledge of computers, printers, or scanners. The system maintains records for up to 30 separate assessment occasions (tests or ratings) per course. Each user may specify up to 40 rating criteria per inventory and 30 inventories with different criteria. The rating criteria used in one course need have no relationship to those used in another. The paper first provides a detailed, generic description of the system followed by descriptions of its application in a Pediatrics Residency and Junior Year medical school clerkships (i.e., clinically oriented courses) in Pediatrics, Sub-specialties, Surgery, and Obstetrics/Gynecology. Each application section contains an example inventory, reports containing actual data, and the user's comments on the usefulness of the system. Experience indicates that the system has greatly improved the quality, quantity, completeness, and timeliness of evaluation information available to educational administrators, faculty, and students alike. The system would likely prove useful in any training or practice setting, health-care or otherwise, where repeated ratings of human performance by multiple raters is needed to insure proper quality control.
