The School Health Assessment, Planning, and Evaluation Project (SHAPE) was a systematic survey to determine the health status of the New York City school population prior to a major reorganization of school health services planned for the 1984-85 school year. It comprised 6,282 pupils attending 12 public and non-public schools. Over 30 percent of children had conditions requiring referral for further diagnosis or treatment. Junior and senior high school students revealed in questionnaires a variety of health risk factors increasing markedly between grades seven and ten, including sex and venereal disease, and use of cigarettes, alcohol, and other drugs. More than half reported serious problems or losses in their lives during the past year and indicated a wish for classroom instruction in sexual matters. Eating habits were found to be less than ideal but similar to those of other American teenagers. Dental screening revealed a higher rate of untreated cavities than that in areas beyond New York City but also showed an overall reduction attributable to water fluoridation. The children were found slightly shorter and heavier than their peers elsewhere. This report includes methods, results, and appendices of statistics and forms used. (LHW)
