The report describes the activities of Model Vision Project, a Tennessee program which diagnosed multiply disabled children and adolescents with visual problems and provided services to those children and their families and workshops and conferences for staff training. The first section provides an overview of the project and descriptions of the diagnostic component, the field services component, the training and evaluation component, job descriptions for each staff position, and populations and area targeted by the project. The second section describes the program goals developed in the planning stages and the means utilized by the project to reach the goals, dissemination and marketing activities, ancillary benefits to parents and other professionals as a result of project activities, and the results of evaluation and validation measures used to monitor and determine project effectiveness. Among appendixes are sample evaluation instruments used with severely handicapped children, sample educational evaluations, job specifications, workshop information, and prevocational activities used in the program.
