This manual has been assembled to enable other Schools of Public Health to survey faculty and alumni to collect information useful in curriculum planning and development. A record system is included. In addition to the "Schedules" on which information concerning study units (i.e. individual faculty or alumni) is recorded, there are "Forms" for keeping track of the study subject and the communication between them and the investigators. The survey instruments, Schedules A-D, are supplemented with a rationale for the collection of the various items of information. A code book is provided for each Schedule along with additional content codes for certain items of information such as job category. Control cards are included that enable the investigator to edit the data in a preliminary fashion and to generate basic frequency and contingency tables for information formatted according to the code books.
