The booklet is designed to show how agencies work together to meet the educational needs of young multiply handicapped children from the time of referral through entry into regular school. Section I on identification focuses on agency contacts, presentations, documentation, and cooperative agreements. A second section covers checklist, the referral form, and referral procedures. A final section on assessment describes screening and testing in various areas, on-going assessment, the individual educational program, treatment team, report cards, and the test matrix. Appendixes are included for each section and provide such materials as the following: child identification agency contact plan, record form, and listing of eligible children; checklist; referral form; parent consent letters; management record form; and progress report forms.
