This packet of forms, instruments, and guidelines has been assembled to aid in the determination of the articulation status and needs existing in elementary-junior high school and junior-senior high school programs. The term "articulation" is understood to mean the smooth transition from one grade or subject area into the next with as little discontinuity and duplication as possible. Included in the packet are: (1) procedural suggestions for any program intended to assess needs in education; (2) a teacher questionnaire, assessing teacher beliefs in the levels of articulation and articulation awareness existing in the system; (3) an administrator questionnaire, surveying the same areas; (4) data collection instruments for questions asked in surveys 1 and 2; (5) an interpretation guide for questions in surveys 1 and 2; (6) a brief outline of a "Management by Objectives" model for implementing educational plans; (7) a discussion of administrators' role in articulation processes; and (8) a discussion of teachers' role in the improvement of articulation.
