This document outlines a system for planning recreation therapy, documenting progress, and relating the entire process to a team approach which includes patient assessment and involvement. The recreation program is seen as therapeutic, closely related to the total medical treatment program. The model is designed so that it can be adapted to almost any health care setting where the value of recreational activities in patient treatment is recognized. It includes six steps: (1) initial patient interview; (2) patient recreation needs assessment; (3) evaluation of medical data; (4) problem identification; (5) goal identification; and (6) progress documents.
