Hundreds of thousands of children and youth with disabilities across the country are receiving secondary transition services. Secondary transition is the process of preparing students with disabilities for life after they leave high school, including participation in post-secondary education or training, employment and independent living. Secondary transition was established nationally as a policy and funding priority for high school special education programs in the early 1980s. The policy was brought on by studies to determine how youth with disabilities who left school were doing--significant problems were revealed. Although federal law mandates that students with disabilities receive transition services from age 16 to 21, Pennsylvania and 34 other states offer an Individualized Education Program (IEP) beginning at age 14. The IEP is the document used to facilitate an individualized planning process during these critical transition years. The purpose is to start communicating with the young person and their family early on to determine what they want to do. According to the Pennsylvania Department of Public Welfare, there are countless resources available to youth with disabilities in the state to help them in the areas of training, education and employment.
